It's a speculum, a device to provide visualization of the vagina and cervix as part of a gynecologic examination. It is a metal speculum, largely abandoned in the era of HIV and other infectious agents with the potential to be spread by unsterilized medical equipment. Most clinicians have been using single-use, disposable plastic specula for many years. These have the added advantage of being room temperature; the metal specula had a tendency to be very cold, making an already uncomfortable examination even more unpleasant.
